Breaking bad news: tackling cultural dilemmas

Abstract Requesting that serious diagnoses be concealed from patients, a widespread phenomenon in many cultures, presents professional dilemma. Practical and sensitive communication strategies are needed. Methodology In this paper, we use analysis of the existing literature to develop tool for practitioners facing requests diagnostic non-disclosure. Our approach builds on strategies, providing mnemonic tool, permitting more than one outcome, focusing need mutual understanding cooperation. Results Existing work dilemma highlights appreciate family's standpoint, affirm their benevolent intentions correct misperceptions. To end, have developed 'ARCHES', used situations where family has requested The model six stages: acknowledge request non-disclosure, build relationship, find common ground, honour patient's preferences outline harm provide emotional support devise supportive solution. Conclusion Facing non-disclosure is challenge communication. particularly marked when practising across cultures. gives structure building rapport with realigning misperceptions while upholding right knowledge.
